If you set the option to 0s and just simply upload a single file on foreground, does it beep?
You can have WinSCP beep, when queue clears, using Beep when work finishes preference option:

I also find myself wishing to know exactly when an edited file transfer is done, without having to look. for me, WinSCP is on a remote machine, whose entire desktop is just a window on my local machine. I receive audio though.
ideally I'd be at my browser, ready to load the new upload at the sound of a beep. instead I wait at WinSCP so I can see when it's done, or wait at my browser and guess when to try.
applying work notifications to background transfers would be fine.
adding a separate notification would be good too.
allowing custom .wav files for different scenarios would be great!

If WinSCP could include option to playback .wav file upon completion/fail. I presume the beep is system default or internal beeper. Also include with installer basic sounds for complete/fail/connection error.
BTW, I noticed a "Beep when work finished, if lasted more than [X] seconds" setting in the "Environment" section of the preferences, under "Notifications" which is where the system tray balloon option is. However, it doesn't actually seem to beep when files are finished uploading (even large ones, let alone small ones, or temp files being edited in an external editor).

Are you tranferring on foreground or background?
Seems like setting WinSCP to make a sound when it finishes work that takes longer than 0 seconds should've done the trick, though it doesn't.
