OK, you are right, this one is not straightforward.
You need to start transfer in GUI, select New and updated files only option, in the Transfer settings dropdown menu select Set as Default and start the transfer.

Can you tell me how to turn off\on file exist in WinSCP GUI? I do not see this option.

Edit 2021: Use -neveronly switch.
For example with the put command:
